Dr. Addis is Assistant Professor, Obstetrics and Gynecology, Director of Urogynecology, Associate Director, Female Pelvic Medicine and Reconstructive Surgery; Board certified by American Board of Obstetrics and Gynecology and graduated from the University of Arizona College of Medicine in 1998. Residency, University of Tennessee; Fellowship, University of California; Recognitions, UCSF Award for Outstanding Patient Service, 2004.
